ABSTRACT

Abstract Eccrine porocarcinoma (EPC) is a rare malignant skin tumor. The dermoscopy of invasive EPC reveals focal presence of whitish-pink, structureless areas surrounded by pinkish-white halos. In an eccrine poroma (EP), such areas present diffuse distribution in the "frog- eggs" pattern. We reported an EPC in situ that presents a transitional dermoscopy pattern between EP and invasive EPC. We found a diffuse distribution; whitish-pink, structureless areas surrounded by pinkish-white halos; a central exulceration and a polymorphic vascular pattern.

ABSTRACT

Abstract: Eccrine poromas and porocarcinomas are adnexal tumors derived from the sweat duct epithelium. However, eccrine poroma is benign in nature, whilst eccrine porocarcinoma is regarded as its malignant counterpart. We report the case of a man who presented simultaneously with an eccrine poroma and eccrine porocarcinoma. Both lesions had no clear distinctive features, enhancing the need of high-level clinical suspicion together with surgical excision and histopathology for prompt diagnosis confirmation.

ABSTRACT

Abstract: Hidroacanthoma simplex is a rare intraepidermal neoplasia that arises from the acrosyringial portion of the eccrine duct. Malignant transformation of hidroacanthoma simplex is reported in the literature and the treatment is performed with wide excision or Mohs micrographic surgery. We report the first case successfully treated with cryosurgery with a long-term follow up.

ABSTRACT

Abstract

Eccrine poroma is a benign adnexal tumour of the uppermost portion of the intraepidermal eccrine sweat gland duct and acrosyringium. Eccrine porocarcinoma is the malignant phenotype arising from the intraepidermal portion of the eccrine sweat gland duct epithelium or from pre-existing eccrine poroma. Both commonly occur in the palms or sides of the feet; these areas have a high concentration of eccrine sweat glands. We describe two respective cases of benign and malignant eccrine poroma on the scrotum, which entailed good excisional results.

ABSTRACT

Los tumores anexiales de la piel representan un grupo heterogéneo de entidades de baja frecuencia de presentación. Su origen es controvertido, quizás a partir de células madres pluripotenciales, aceptándose principalmente dos líneas embriológicas: pilosebáceo-apócrina y ecrina. Se los clasifica en cuatro grupos principales: tumores del folículo piloso, tumores sebáceos, tumores apócrinos y tumores ecrinos. Su distribución anatómica refleja áreas con mayor densidad de anexos cutáneos, presentándose en forma única o múltiple.


Skin adnexal tumors are represented as a heterogeneous group of entities of low frequency range. Its origin is controversial, perhaps from pluripotent stem cells, mainly from two embryological lines: pilosebaceous-apocrine and eccrine. They are classified as four main groups: tumors of the hair follicle, sebaceous tumors, apocrine tumors and eccrine tumors. Their anatomic distribution reflects areas with the highest density of cutaneous adnexal, arising in either single or multiple forms.

ABSTRACT

Con el objetivo de describir las características clínico-patológicas de los casos de porocarcinoma ecrino (PE) registrados en el Instituto Nacional de Enfermedades Neoplásicas (INEN), se realizó un estudio trasversal que incluyó todos los casos entre 1998-2009. Para ello se realizó una revisión de fichas clínico-patológicas y una nueva lectura de las láminas de patología de los casos encontrados. Se hallaron 19 casos de PE; la mediana de edad fue de 64 años (rango: 37-98); con mayor frecuencia en el sexo masculino (57,9%); la localización más frecuente fue en el pie (21,1%), seguida de cabeza, tórax y muslo (15,8%, en todos los casos). Se describe un caso de localización glútea que cursó con una conducta inusualmente invasiva y con compromiso rectal. Se concluye que el PE es una entidad poco frecuente en el Perú, su localización más común es en el pie, y afecta predominantemente al sexo masculino.


In order to describe the clinical-pathological characteristics of the cases of Eccrine Porocarcinoma (EP) found in the Instituto Nacional de Enfermedades Neoplásicas (INEN), a cross-sectional study was carried out, including all EP cases recorded between 1998 and 2009. A revision of the clinical-pathological records indentified and a new reading of the pathology slides of the cases found were performed. 19 EP cases were found, the median age of which was 64 years (range: 37-98). EP was primarily found on male patients (57.9%) and was most commonly located on the foot (21.1%), followed by the head, thorax and thigh (15.8%, respectively). In one case, EP was located on the gluteal region, and showed an unusually invasive behavior and rectal involvement. We conclude that EP is a rather uncommon entity in Peru. Its most common location is on the foot and it affects mostly male patients for the most part.

ABSTRACT

O porocarcinoma écrino é uma neoplasia maligna, rara, das glândulas sudoríparas écrinas e, com maior frequência, acomete os indivíduos idosos, com idade média de 67,5 anos. Apresenta manifestações clínicas variadas e sua localização não se correlaciona com a concentração das glândulas sudoríparas, ocorrendo, principalmente, nos pés e pernas. O seu diagnóstico e tratamento devem ser precoces para evitar a agressividade do tumor, que ocorre em 20 por cento dos casos. Os autores relatam o caso de um porocarcinoma extenso no tórax, em paciente do sexo masculino, de 71 anos.


Eccrine porocarcinoma is a rare, malignant neoplasm of eccrine sweat glands. It often occurs in elderly people, mean age of 67.5 years. The clinical features are variable and their localization is not related to sweat-gland concentration, occurring mainly on the feet and legs. Disease diagnosis and treatment should be as early as possible in order to avoid tumor aggressiveness that occurs in 20 percent of the cases. The authors present a case of extensive porocarcinoma on the thorax of a 71-year-old male patient.

ABSTRACT

A 64-year old male patient presented to the dermatology clinic with few weeks history of multiple papulonodular skin lesions over the left thigh, associated with limb edema and palpable lymph nodes in the left inguinal region. The patient had a skin lesion over his left ankle, excised 7 months earlier and the pathology report came with the diagnosis of squamous cell carcinoma. A biopsy was taken from one of the thigh lesions, and histopathological examination revealed metastatic porocarcinoma into the overlying skin with lymphatic infiltration. A histopathological review of the first biopsy showed a porocarcinoma, apparently completely excised. In conclusion, this case presentation highlights the tendency of porocarcinoma for local recurrence and metastasis to skin and lymph nodes

El poroeorcinoma ecrino es un tumor cutáneo maligno infrecuente que deriva de las glándulas sudoríparas ecrinas y que afecta fundamentalmente a personas de edad avanzada. Se localiza con mayor frecuencia en extremidades inferiores, cara y cuero cabelludo y son potencialmente agresivos, tanto localmente como a distancia. Reportamos el caso de una paciente con diagnóstico de Porocarcinoma Ecrino Pigmentado en cuero cabelludo, que fue tratado con extirpación total de la lesión.

It affects primarily older people and is found more frequently in lower extremities, face and scalp, and is potentially aggressive, both locally and through methastasis. We describe o patient diagnosed with pigmented eccrine porocarcinoma of the scalp, and treated with total excision of the lesion.

Porocarcinoma écrino é um tumor de pele raro derivado do acrossiríngeo. Quatro casos de porocarcinoma écrino são estudados. Os pacientes tinham idade entre 49 e 64 anos, sendo dois homens e duas mulheres. O tempo de crescimento dos tumores variou de um a 20 anos. As lesões mediam de 1,2 x 2 a 4 x 5 cm e se localizavam nas extremidades ou na face. Dois pacientes apresentavam carcinoma basocelular em sítios diferentes do porocarcinoma écrino. Duas das biópsias foram previamente descritas como poroma écrino; entretanto, malignidade foi observada em todos os casos após excisão ampla. Portanto, em toda biópsia de pele compatível com poroma écrino, deve-se considerar a possibilidade de malignidade.


Eccrine porocarcinoma is an extremely rare skin tumor derived from the acrosyringium. Four cases of eccrine porocarcinoma are described, two males and two females, aged from 49 to 64 years. Tumors grew in an interval of one to 20 years. Lesions ranged from 1.2 x 2 cm to 4 x 5 cm and were on limbs or face. Two patients had basal cell carcinoma in a site different from the eccrine porocarcinoma. Two biopsies were described as eccrine poroma. Malignancy was observed in all cases after wide excision. Therefore, eccrine porocarcinoma should always be considered after a biopsy compatible with eccrine poroma.
